I assume there's a few ways, but I got it when I had Refresh and two Steam Sales.

The wiki says that you need to get 50 items (through any means, like 2 steam sales and restock)

I got it on a Hard run, nothing special, no restock, no D20 etc.

I just got lucky. Takeable Devil deals each floor (Found a joker card Floor 1 and 3, after that, the RNGesus was on my side it seemed), loads of HP ups, Double-bought two store items with Diplopia, found 3 Libraries, got 2 bloodbags from arcades, got Pandora's Box for two more items, literally found every available head item in Golden Chests before I even got to the Depts.

By the time I got to the chest, it took like 4-5 rooms with chests for me to unlock it.

I swear to God, I've never had such a lucky run in my life.

I accidentally broke U Broke It. I got it on a victory lap. Technically you're not supposed to get achievements on victory laps, but apparently some of them still work.

It's easy actually. Play as Azazel, get D20, wait for the floor with a little battery for sale and on this floor complete every battle without picking up any money. Then start rerolling, and some consumables will inevitably turn into chests which in turn will give you even more consumables. The scheme is simple - reroll, get a battery from the shop, return to the room, open all chests, pick up the most valuable consumables (golden keys, maybe trinkets, nickels and dimes), and repeat. Soon you'll have so many stuff that the whole room will be full of them, and that's when you can start buying items because on every reroll you will get even more money from the chests.

Its better to be playing isaac for a d20 break. Not just for the d6 rolls but the 1 bomb at start can improve your odds a lot of winning a greed mode. Cain is also an acceptable choice as gold chests he can open for free.

Don't remember how I broke the game when I got it, but my favorite method is Blank Card + 2 of Diamonds on Greed mode or with Restock.

Don't forget Cain's key. With him you can get into the Treasure room right away. Helps to determine wether to restart the run or not.

Well, I'm not that experienced in breaking and chose Azazel because, even if you have nothing good besides D20, you'll be able to clear first 2-3 floors without too much problem. But now, when I think about it, Cain definetely seems like a good option too. As for Isaac - I don't know, his D6 won't be able to do much before you change it to D20/Blank Card.

I just got it today, I can confirm you CAN get it on a Victory Lap, I did it myself.
