How rare?!
I've restarted 1,000 times and I still haven't gotton Tech X,moms knife,deaths touch,spoon bender,holy mantle,proptosis, or polythemus... I know these items are rare but just HOW rare?

It's less about rarity and more about the sheer number of items in the pools. That, combined with an item being rare, means it's going to be quite a few runs before you find these items in all likelihood. But it does make it feel really nice when you do find 'em.

OMG I've spoted the scrub lord. Maybe you also want over 9000 damage, infinitive holy mantle and a press to win spacebar item?! AND CALL THIS GAME THE BINDING OF EASY: REBIRTH OF NO ♥♥♥♥♥♥♥ SKILL!!!!

I have 1001% So it's going to be a ♥♥♥♥♥ but I just want no NEED Tech X for deleruim (Keeper)

No but after losing to deleruim for the 5th time as keeper due to not being able to have enough luck against his teleportation on top of you I just want a good item to start off with. Or, do I have to play all the way to the depths 2 with chemical peal being my only damage up? Or, better yet why not take deals with the devils then get nothing but speed and range ups? Sounds like a fun time right? Oh did I not mention my luck when playing the keeper is about as much as his -2.

... If you managed to beat Hush as Keeper, you should be good enough to kill Delerium. With teleportation on top of you - that's developers problem. Don't bother with this character and The Lost untill they fix that.

I've finished ultra greed and deleruim as the lost. But I should've probably just try to finish other unlocks for now...

Mom's Knife and Polyphemus are both considered Special Items, meaning they're rare to begin with and running into other Special Items lowers the chances of getting other Special Items. As for the other items you've mentioned, it's most likely having poor luck. I have the same issue with Brimstone. Never around when I want it.:steamsad:

The answer is... a bit technical actually. You should read the "special item generation" part of the wiki page. Here's the link: http://bindingofisaacrebirth.gamepedia.com/Special_Item

Rephrasing from the wiki:
Those items are not rarer as any other as long as you never saw any on your run (rerolling a held item into a special one does count as "seeing one"). After seeing one, they are twice rarer. After seeing 2, 10 times rarer. After seeing 3, 50 times rarer. Afterwards, 100 times rarer.

That's only about special items of course. Other (regular) items are as rarer as any other item (from the same pool). Thing is, some items are in several pools, so you can see them several times in one run (obviously more times than an item which is only in one pool). I'm not even considering the "forced drops" items... :P

Anyway, couting on some specific items to win a run isn't a good strategy, unless you want to spend hundreds of hours in it and still be "meh" at the game. There are already tons of threads about "how to get better", so I won't bring those tips here.

Odd, I still manage to get items like dr fetus commonly though I find it mostly unusable sure the damage and ability to break rocks pots and shrooms is good but fire rate and explosive damage just makes it unreliable... I don't RELY on special items I'm sure I could beat the hush with keeper if i got some damage upgrades and maybe a good trinket but deleruim is too unpredictable for me not to try for OP items also thank you for wiki.

How rare? There are 400+ power-ups in the game. Only a fraction of them will be in the item pool each run. Only one item from the pool will appear in the first item room (maybe two if you're lucky) and the odds of it being rare are, well, rare. So yeah. The odds are against you.
