it changes the appearance of Isaac, randomly makes certain visual effects you have from items disappear.

Yeah and that the answer could be more complete.

On destruction it drops Pills 1-4, except you got the right modifier , then it will drop cards instead.

I think that they make your face back to "normal". Some items like growth hormones, that makes some balls in your face are removed (not the item, the cosmetic of the item).
