Error trinket question

i have the error trinket, along with: my shadow, mom's knife, bum friend, shielded tears, brother bobby, missing no., lost contact, rotten baby, and moms pad. I took damage in a room, and the D12 effect occured. I'm 95% sure it was the trinket, but I don't know which trinket it was that did it. Any answers?

Sounds like 404 gave you the Cracked Dice effect.
