There's no real problem.
Longer version:
Last year, Devs changed the Keeper sprites to look like the Old Man Keeper for April's Fools day. Old Man Keeper was a red herring for dataminers when Afterbirth came out, keeping the real Keeper a secret.
The game (probably) has a built in .xml file equivalent in its .exe to manage this menu for this specific day. Since it was created when Afterbirth+ didn't exist at all, and this file equivalent never got updated for Afterbirth+, this part of the code doesn't know what is Apollyon. Since there are no lines at all for Apollyon, the game simply don't show it in this menu, so there's an empty space.
Nope. How do I know? Set your date settings to a different day, menu won't change. Go into offline mod, menu is normal again even if you're date settings are on April 1st.
This has to be some command Isaac is getting from the dev servers.
But some one forgot to patch the April 1st stuff, that's for sure. Could have done that pretty smoothly alongside the booster pack. Failed opportunities eh...
