Question about Shop item pool

When I see item for sale in the shop, is it removed from item pool? Or only if I buy it? And what if I reroll that item without buying?

An item is only removed from the pool when you pick it up. The same goes for shop items.
The item can reappear in other shops if you choose to either reroll it or simply not buy it.

Almost.
Its weight is decreased for the current pool as soon as you see it (often removing the item, but in some pools it just becomes rarer).
If the same item exists in several pools, when you pick it up (no matter from which pool), it is completely removed.

Indeed, some items do not follow that rule. "Forced generation" items aren't removed at all, and the Bible is quite special (it can appear almost anywhere, even if already picked up once).

That explains why the bible only shows up where you don't want it to.
