Broken Gaping Maw
This is the final missing entry into my bestiary. I've got the game 100% completed with exception of this guy.

Does anyone have a seed perhaps where he's located? Maybe he'll show up for me then (Although I expect it doesn't count that way tbh). I've had a lot of games in the past few weeks, fully exploring the Womb but I'm never getting him.

Yep. I was kind of surprised when the achievement popped! Thanks man, I'll let you know if it worked for me :)

Wow! This is a really good seed.
First Devil Deal was Dead Cat. At 1 red health, I decided to go back and check out the Boss Challenge Room. 3 Red Chests, each with an item - one being Guppy's Head, another being Guppy's Tail. All 3 Guppy items on the second floor? Okay!

Correction: Apparently I was playing normal, not hard. No Boss Challenge Room in hard mode. Also, no Broken Maw in normal :(

Did you get the two pieces from the second floor Boss Challenge like I did, or did you get them later in the run? Are the Challenge Rooms randomized?

Exactly as how you described it. Cleared floor 1. Got Devil Room on Floor 2, got Dead Cat (Even saved the Hierophant card) and then I backtracked, went into the Challenge Room and got 3 items from the triple chests completing my Guppy Transformation. Third item was Plan C iirc. I also found Brimstone on floor 3 and a Crawlspace with a Black Market, bought everything there. Kind of an easy OP room. And it helps a lot to become Guppy :)

And thank you for this! Just finished the run and found the Broken Gaping Maw. I've got this game finished full 100%. Here's hoping we'll get another Isaac game or DLC in the future :)

Weird... I played again on hard, but no Challenge Room. I guess the map is randomized too. Now I want to replay this and see if I can pull that off again.

I just tried the seed, hard mode as Isaac, Broken Gaping Maw didn't spawn on womb2 for me. maybe it's patched since post? I went in every room on the floor, but didn't find it :(

Seed generation changes every time something is added to the game.
Look at the date of those posts. Several booster packs got added in the meantime, those seeds are outdated.

Weird thing is i played it and got 3 guppy items in the boss challenge room as they described should happend but i never found broken gaping maw on womb 2 which is really strange
