Name: Heart Power (if you have a better name let me know)
Description: Put your hearts into it!
Appearance (item pedestal): A heart with wires in it
Appearance (on player): Wires going from a device on the player's head to the area where the heart normally is
Item Pool: Angel Room (somewhat rare chance to be found)
Effects: Effect depends on what heart type the player has the most of. If it's red, the player will spawn 1-3 friendly hearts when hit (with a 5% chance to spawn a mask with each heart). If it's soul, the player will have the Wafer effect and will gain the effects of either Holy Mantle (10% chance), The Bible (40% chance), Holy Light (9% chance), Sacred Heart (1% chance), Destiny Spear (20% chance), or Holy Water (20% chance) for the current room when hit. If it's black, the player will gain The Empress effect and gain a one room damage up on hit. If it's empty, enemies have a 1 in 20 chance to drop red hearts upon death (does not effect regenerating enemies or dark red champions), and the player gains the tick effect (minus the part of it that removes the ability to swap trinkets).
Other: If playing as the Lost, it will use the Empty effect. If playing as Keepeer, it will give him coin tears. If modded hearts are the majority, it will give the player the Soul effect.
I did consider some other effects like Holy Light and Boogers, but they would make it overpowered. Fire and Explosive shots would make it more of a risk/reward item, which could make it more interesting.

Type: Active (6 room charge)
Appearance: A pink bar of soap.
Item Pool: Item Room, Shop
Effect: On activation, Isaac holds the Soap above his head. If a firing key is pressed, it will be used in whichever direction was pressed. Upon use, it will fire a random Monstro's Lung-like spread of 5 slow moving yet long lasting bubbles that come in random sizes, with the size displaying the bubble's "bubble stat" (bigger is better).
If a bubble touches an enemy, one of 2 things can happen. If the bubble's bubble stat is too small in comparison to the enemy's total health, the bubble will pop, releasing 4-12 little bits of bubble juice that do a tenth of Isaac's tear damage in random directions.
If the bubble's bubble stat is just big enough for the enemy's total health or bigger than the enemy's total health, the enemy will be absorbed into the bubble, and the bubble will not be able to damage any more enemies, as well as gaining spectral status. When this happens, the bubble will try to float off screen. Pop it for a free Soul Heart (it might taste a bit odd though).

Effects 1+ damage upgrade. All enemies in the room have fear effects for ten seconds.

Looks like big, old, red alarm bell with hammer. Isaac's Head becomes School Bell while held. Bell-Head rings when used.
On use, opens all Doors in current Room and all Enemies in the room stop attacking and move to the closest door. When an Enemy reaches a door, it dies, but Enemies can still deal contact damage while moving to the door.
If used on a Boss, if the Boss is at 25% Health or less, the Exit opens and the Boss gets sucked into it; again, still dealing contact damage if Isaac is in the way. If Boss is at 10.001% Health or more, the Exit opens and Isaac gets sucked in, skipping the Boss fight, but also the Boss Item.
Damn! Very complexive!
Well, I figure the point of making a modable game is to modify everything so... why not get creative?

Passive Angel/Secret Room Item.
Looks like Isaa'c head, with Isaac's head for eyes, with Isaac's head for eyes, with... Plays audio clip of: "Isaac and his Isaac lived alone in a small Isaac on a Isaac" when picked up.
Grants 1 Extra "Alternate" Life, but when the player dies everything is re-rolled. Like, really, EVERYTHING...
First, the "Character" is re-rolled, but not like with the Clicker. Only the character's appearance and starting abilities change, not the character's health. The player restarts at the starting health of their starting character like with the 1 Up! mushroom (before items are re-rolled, below), so if Isaac, 3 full Red Health.
So you could re-roll as Lost, with the Holy Mantle, Flight, Spectral Tears, AND actual Health! But if you pick it up as Lost, you could end up as Maggy with no Health, no Mantle, and a useless Yum-Heart. Note that this re-roll will override activated items, so if Isaac becomes ???, goodbye D6 and hello Poop, but still, you'd be a ??? with Red Health, which is something different at least.
As for achievements, you are the character you become, which means you could be Lost or Keeper with actual health. It's a very slim chance of making some of the worst achivements more achievable.
Second, the Seed is re-rolled to a new random Seed, then the Floor is reset, like with Forget Me Now. The player respawns in the Starting Room, but of a completely different Floor, so Caves II might be re-rolled to Flooded Caves II, by the new Seed.
Third, all collected Pedestal Items (excluding starting items) are re-rolled, D4-style, according to the items available in the new Seed, by number. So if you've collected 11 items at that point, you get 11 different items. But there are no limits on this. D4 and Missing No. seem to give mostly Treasure and Boss items. This will give you ANYTHING from the item pool of the new Seed, from Devil to Angel to Secret Items.
Fourth, if you have a Trinket, it's re-rolled. If not, you still have nothing.
Finally, all Pickup Items are totalled, then re-rolled. If you have 15 Coin, 6 Bombs, and 4 Keys, that's 25 Pickups total, which may be re-rolled into 7 Coin, 0 Bombs, and 18 Keys.
Yup. Re-roll EVERYTHING! (upon death.)
