Keeper's Table.
A Shopkeeper, seated at a table, with a Key and a Bomb on it. Works like the Shell Game guy, that you walk into it to use it, by touching either the Key or the Bomb. Allows you to sell Keys and Bombs for 2 Cents each.
If used enough times in a run (randomized), may self-destruct (like Slot Machines, etc.), giving one of the following:
1 Penny (most common), a Troll Bomb, Store Credit, A Quarter, a Homing Troll Bomb, Credit Card, A Dollar, Head of the Keeper, or Steam Sale (most rare).
Type: passive (like the quarter)
Effect: Gives 75 cents
Desc: Maple syrup, eh?
I think I figured out a way to avoid hard-code to give the Lost, Keeper, and ??? actual Red Health. I'm not entirely sure if this will work, because I don't know exactly how characters are coded, but it might be possible to make a separate character file, like Dark Judas. Dark Judas seems to be a separate character by stats and appearance, but it still unocks Judas' achievements.
It could be possible to create LivingLost, LivingKeeper, and LivingBaby characters that use all the characters' animations, have their items, and cue their achievements, but still allow for actual health by not using the same character file.
Can someone who makes mods tell me if that's doable?

Appearance : A Baby Familiar wearing armor and holding a small shield and lance.
- Floats in front of Isaac in whatever direction he last fired a tear from, pointing his lance and shield forwards as long as Isaac isn't firing.
- Hovers closely behind Isaac while he is firing.
- Deals 2 damage per tick to anything touching Knight Baby normally, and 6 damage per tick to anything touching Knight Baby's Lance.
- All of Isaac's familiars can now block tears.
- Can block brimstone & technology lasers while Isaac isn't firing.

Toy Robot
"What can it be?"
Appearance : A blocky red, white, and blue action figure shaped like a robot.
- When used, Isaac temporarily gains a random transformation he doesn't already have for the current room.
- Stat changes & red heart containers gained do not persist between rooms.
- Passively adds an additional copy of all transformation-inducing items in each valid item pool.
Type : Active
Recharge Time : 6 Rooms

Mouse Cursor
"It clicks with you."
Appearance : A regular mouse cursor with a black outline.
- Gain a cursor familiar that automatically stays on top of any enemy in a room, dealing 1 damage per tick.
- Attempts to drag any enemy it's on top of away from Isaac.
- Drags collectables towards Isaac if there are no enemies in a room. Can carry items through obstacles and above pits.
- Can be controlled with your actual mouse cursor.

Hungry Kid (Transformation)
- You heal 1/2 a red heart whenever you pick up any kind of heart pickup.
- Full red hearts can now restore half a soul heart or sin heart if your regular heart containers are full.
- All items that increase HP but don't affect any other stat now grant several stat boosts.

Mega Baby (Transformation)
- All enemies are feared for 3 seconds whenever you enter a room.
- +1.0 damage up
- Contact damage with enemies deals 1/2 a heart in damage.
- Is applied while you have 3 size increases on Isaac. Goes away if your size is decreased below that threshold.

Tiny Baby (Transformation)
- All enemies are charmed for 3 seconds whenever you enter a room.
- -1 Tear Delay (Tears Up)
- Is applied while you have 3 size decreases on Isaac. Goes away if your size is decreased below that threshold.
Type: Active (4 Room Charge)
Description: Bury the darkness
Effect: This item will destroy all rocks/skull/poops/etc... in a room much like the Hagalaz rune; however, if their is a hole (pit) adjacent to the rock destroyed, it will be filled in. (A single rock is able to fill in multiple holes, it doesn't select one over the other.)
Synergies:
Item Pool: Shop, Treasure, Secret Room
Item Apperence: Would look just like "We Need to go Deeper" but mud would reside on the head, with a mud palm print on the shaft and mud finger prints on the handle where the hands would grip.
Name: Mind Stabber
Type: Transformation
Appearance: Isaac gains a giant knife through his chest.
Items that can get this transformation (3 required): Wire Coat Hanger, Guillotine, Mom's Knife, Sacrificial Dagger, Betrayal, The Mind, Backstabber, Spoon Bender, Crown Of Light (Those tears are pretty sharp looking ok?) (there may be more stabbing-related items I missed, if so, please tell me).
Effect: Upon taking damage for the first time in a room, Isaac fires out 8 tears similarly to Varicose Veins or Tammy's Head, and the knife flies out of his chest, giving him a knife familiar similar to a Ludovico Mom's Knife, but with must less damage. It is still controlled the same way as the Ludo Knife, and gives enemies the bleed effect.
Synergies: Ludovico Technique + Strange Attractor (the knife will constantly be attracted to the center of your tear, effectively making it a damage up), BFFs (increases the size and damage of the knife).

Type: Passive
Item Pool: Treasure Room
Effects: When firing there's a chance of a zig-zag electric tear that if it hits an enemy it shakes up the current room and kills all the enemies in the current room. If a boss gets hit by this special tear they will lose 15% of their health.
Randomly (10%) the Devil Room will have an Angel Statue instead of the Devil Statue.
Instead of Devil Deals, the Angel will sell Angel Room items, in exchange for Soul Hearts. Better items will cost 2 Soul Hearts, while the less useful items will cost 1 soul heart. He will not accept Red Hearts.
If the statue is bombed, it will spawn the dark versions of the Angel from the Mega Satan fight. Upon defeat, he will drop the opposite Key Piece that the normal Angel would (Key Piece 2 for I versions of floors, the Key Piece 1 for II versions)
EDIT: I just realized that this thread is only for items... I still like the idea, though. If nothing else, it makes Angel Items more accessible.
"Do you feel lucky?"
- Whenever you get no reward from a Slot Machine or Fortune Teller, automatically re-roll once for another chance.
- +5 Luck for the purposes of activating the effect of one of your passive items.

Activated Shop Item, use any time. Costs 30 Cents; 15 on Sale, 7 for Sale on Sale, and still 0 with double Steam Sale.
Looks like a red robot toy on pedestal.
Isaac now has 3 forms: Robot, Car, and Helicopter, which can be cycled (1, 2, 3; 1, 2, etc.) at any time with the spacebar.
1) Robot Isaac (starting form) gets +2 Base Damage and -0.2 Speed.
2) Car Isaac has Speed 3.5 and is immune to contact damage (including fire and spikes) but not to projectiles, and cannot attack except by ramming enemies (like Aries). This works both as an almost-Gnawed-Leaf for non-projectile enemies, and as a way to back-track without wasting too much time or hitting spikes you forgot about.
3) Helicopter Isaac has Flying and gets -2 Fire Rate (breaks cap), and +0.3 Speed.
Note: Transforming is a 1/2 second animation which leaves Isaac volnerable.
