Most boring item?
For me its something like Moms underwear. Its been the same for years and it has nothing specal. Sure a range up can be useful for Azazel but...

I've looked at all the items in the game and this is what I can tell for each item with my own 670+ hours of experience and to speak for everyone with causion; what's useless, what's boring, and what's both (and sorry for the long list).

Active:

D10 - I mean, I don't know if it's a boring item, but damn sure useless and just pointless like, why would you want to reroll ememies in the current room? To have a small challenge on the first levels or something?

Dead Sea Scrolls - I'm not even sure if luck works in this case. You can only hope in your worst situation to be saved or have something to drop something that you really need... anyway: Dead Sea Scrolls < Metronome (unless Metronome activates passive items)

The Hourglass - I'll just put it this way: The Hourglass < Mom's Bra (that's when I got both items more often on the original game)

Isaac's Tears - as boring as it is (regardless of synergies, but as boring as it is), Ipecac will make life beautiful.

Mom's Pad - not the worst item but definitely boring.

Scissors - has anyone even tried to concentrate with their head separated having great build?

Unicorn Stomp - ARGH!

D12 - unless you get tinted rocks, mushrooms and skulls, it's really underwhelming.

Kidney Bean - I just wanna restart!

Mine Crafter - basically it's a free bomb, but boring...

D1 - this little fella isn't helpful as I thought.


Passive:

Aries - except for its speed boost, underwhelming.

Cain's Other Eye - if there's anything that isn't worst than than this item!

Every Food item that only gives you health - they're helpful, all right, but we all need a good stats-up items after our boss fights.

E Coli - yawns and restarts

Forever Alone - worst Fly item on the list?

Guillotine - this is the worst item for me! Despite the stars-up boost, it's a game changer and decreases your concentration when moving. It's annoying.

All the familiars that don't have any special tear effects (with the exception of Cain's Other Eye being on the list).

Mom's Eye - meh...

Roid Rage - if not the most annoying syringe om the game!

Sissy Longlegs - I asked for a useful familiar - gets a wanderer instead -_- (oh, hi, Steve!)

Spelunker Hat - it's helpful, but feels boring.

Strange Attractor - Just no...

Wooden Spoon - ...

Betrayal - I don't remember when was the last time I actually agreed to play with this item. When the animation is played after getting triggered (hit) is so cringy I just wanna restart!

Milk! - HE NEED SOME MILK - NOT!

No. 2 - I can't even describe what an abomination this item brings without having any particular synergy...

The Wiz - assuming the Conjoined Transformation will save your run?

Linger Bean - what the majority says.

Mom's Razor - never find it useful.

D1 can actually lead to some serious game breaking combos, with Car Battery and a room with a battery on the ground you can get infinite batteries then just find any useful coin/bomb/card/pill/spirit heart/black heart and you can get very powerful. also if you find a charged key you can get infinite keys without any synergies whatsoever. I've had it save some greed runs by being able to copy Jera runes or 2 of Diamonds or just a bunch of hearts. also if you find a dime and have a shop with batteries on greed mode you can net 5 coins each time which slowly will give you infinite money

also No. 2 is actually amazing in Greed/Greedier mode as it can clear entire waves each time. with a little bit of practice I found it to be one of the single best starting items in either mode hands down. even in normal runs it means infinite bombs which is invaluable and the minor downside can be completely mitigated by just not holding the fire button

In my opinion it has to be any basic health upgrade, or maybe We Need to go Deeper.
