Is even worth still pick up pills?
Dont get me wrong, some pills are great, but i dont remember the last time that i have seen a "Tears up!" "Health up" or "I can see forever!"

The pills pool is just to big or is it me?

Theres good pills like I'm drowsy and gulp. Though some people do pick to be super boring and not take them at all.

Some pills can carry a run (Gulp, I'm Drowsy, Tears Up). None can destroy it.

There are situations where a pill can kill a run.

Telepills is the only one, that i can think of, as long as you eat your pills out of combat.

Having 1 hp left inside a curse room and taking I'm addicted pill. I'm excited then a really bad boss fight/room/greed mode (Try I'm excited delirium). Bad trip can cause you to miss a vital devil deal that leads to you losing a run. ??? can kill an achivement run by making you miss boss rush or hush, Health down can make you lose a key devil deal. Tears down can make you fire so slowly you can't clear rooms without taking damage. One makes you larger + magic mush can cause you a lot of dumb damage. Horf can kill you if your on your last hp. Explosive pill can make you accidently spawn the angel if you use near the statue and you might die to it.

I know what you mean, the pills are somewhat diluted but they're still worth picking up imo.

I used to take unidentified pills.

I don't any more.

With nearly all of them, the risk is bigger than the reward.

I think afterbirth + diluted the item pools with a lot of average items but they beefed up pills and trinkets for balance. Gulp alone makes it worth the risk to use pills, plus there is a transformation linked to 1 of the neutral pills too

I use every pill every time. I find that it usually helps me win in the long run. Don't get me wrong, there's been a couple of times where teleport has screwed me over. Or I hematesis when I want to enter a challenge room, or full health when I want to enter a boss challenge room... things like that.

Pills make it fun, the one I absolutely hate the worst though... luck down. Everything else is fairly easy to compensate for. Luck is hard to come by.

Oh yeah here's another one that's annoying, getting health up when you have empty vessel. Arrghh!

The key is to know when it's safe to take a pill and when it's not. If you're in a bad spot, don't take pills you don't know, unless you've grinded out the floor and are desperate.

You also shouldn't walk into a curse room with low HP.

Health Down is actually more likely to ruin your build, if you find it lategame and are at zero red-heart containers. At 1 red-heart or lower, it's a Health-Up. The effects of other stat down pills aren't significant enough, since you're only eating them once.

You're Excited? Wait 30 seconds. One Makes You Larger does not affect your hitbox.

curse of the labyrinth? understandable.
curse of the maze? a bit annoying, but understandable.
but curse of the lost? why is removing the map from the hud challenging?

Oh yeah amnesia pill can really mess you over if your trying for boss rush/hush. Or even wind up getting you killed if your under powered on a floor.
