The rule of thumb with the clicker is throw the POS away instantly, unless you recently got an item you want gone. Even then each click you risk losing all your hp for good. This is easily one of the worst items in isaac. It can instantly end a run.
In isaac you are never meant to get every item in a run.
Wasn't really trying to get every item, just hoping to roll Eden's Soul since it's so annoying to find on it's own. That and Tonsil. >.>
When the game has to generate an item but there are none left in the pool, it generates a breakfast instead. You can consider it as the default item for every pool.
Since the item tracker thing can show every item only once, even if you have 30 breakfasts, it will appear as only one. So if you deplete all available pools by rerolling your items, they'll all turn into breakfasts at some point.
That's why you should stop rerolling yourself if you see one, because it means your items will probabaly only vanish afterwards (because they'll turn into more breakfasts). It doesn't ruin anything, it just shows you that you should stop now. :P
