Try to get Flight. You can hide in rocks, while your familiars deal damage.

The Blue Flies damage scales with Isaac's Damage.

Look or Lil Brimstone in Devil Rooms.

Use Dad's Key to your advantage. Open all shops, arcades, secret rooms, anything, in order to offset the lack of treasure rooms.

Aim for flight, and any items you'd get blue flies/spiders from, e.g. Guppy transformation, Hive Mind, Mulligan, Infestation (1 & 2), etc.

Little C.H.A.D. and/or Dark Bum can help out a lot, especially if you have both of them.

Lil' Brimstone would be the most preferred familiar. Try to stay away from Bob's Brain.

Try to get some orbitals. Slot Machines can give you pretty flies.

Avoid Dice Rooms, as rerolling yourself can ruin the run.

Get sacrificial dagger and remove all red hearts for polaroid. Sacrificial dagger with BFF becomes the strongest orbital while having no red hearts allows you to activate the shield whenever you get hit so you can recklessly do massive damage.

I finally did it with lil brimstone and dark bum :D . Just retry until you get a decent devil deal or random pickup.

Hold R until you get a library with Book of Revelations or Satanic Bible. then get lucky on a devil deal

One advice is to do two successful runs in the normal game with Lilith so you can unlock incubus, box of friends and a few other of her items. The challenge is tricky but try to pray you get a good Satan item or find a trap door that contain daddy long legs.
