Its also a billion times more common in my experience.
I guess Nose Goblin is a trinket instead of an item (presumably has a lower activation chance by default).
Edit: Ok that makes a lot of sense now that I thought about it.
And that just sounds an insanely lucky combination, yeah sinus infection probably helped greatly, but you could say that about sinus infection + almost anything that gives you lots of tears/ rate of fire/ Luck.
Besides, nothing wrong with having a strong item in the Item room pool :p (Tech X is in the item room pool and that's practically a run winner just by it's self)

The amount of Boogers on one enemy may make up for that. If it's dependent upon luck, with enough, you could probably start to shred.

The main problem with Soy Milk is that while it quite equally exchanges damage for a smaller tear delay, slightly in favor of tear delay, it just is an advantage when the tear delay is at the standard 10 or higher. Making it 1 at standard tear delay instead (of 2) due to the flat tear delay reduction of 2 after reducing it to 1/5th of the original value like the damage.
The poison/bogger tears will still deal 1/5th of the damage and might be fired just at thrice the rate when you'd actually have had a delay of 5 anyway
for reference, at tear delay 1 (nothing can go lower than that), one fires 15 tears per second, at 5 (soft cap) it is 5/s and at 10 it is 2,72 (with 72 is repeating by the by). One can break the soft cap of 5 with the following items:
Capricorn, Cricket's Body, Guillotine, Mom's Perfum, Pisces: -1
Anti-Gravity, Cancer (Trinket), Milk after taking damage: -2
Purity (Blue Aura OP, better nerf) and Cancer (Trinket) with Mom's Box: -4
best is when you have 20/20, bookworm, incubus and APPLE! with it on lilith :D
aaaand yeah it is a solid item, and you have to hit them and stack up the nuggets. 10x better than explosivo, don't complain :D
Okay... you can't compare Sinus Infection to Explosivo in terms of usefulness. In terms of power and effect, sure, they have similar effects that do lots of damage, but Explosivio explodes your face, while Sinus synergizes with all kinds of things and does continual damage. Sinus is clearly superior.
Boogers are hands-down the best tear-effect in the game and the best item for synergies. That said, I'd still rather have Tech X. Who needs synergies when high damage will clear a room in one shot?
It's really strong, for sure, and I'll take it any time, but the same is true for Tech X, Brimstone, Cricket's Head, Cricket's Body, Magic Mushroom, Polyphemus, Crown of Light, Sacred Heart...
Also utility items like Steam Sale, Restock, Sack Head, Blank Card, Car Battery, Gnawed Leaf, Pinking Shears, Stopwatch, Pyromaniac, There's Options, More Options...
There are a lot of really powerful items in the game, because without them, it would be practically unplayable. You need at least some damage ups to win. Sinus Infection is really, really sweet, but not the most OP thing in the game. Hell, it can't even break the game. Blank Card can be way, WAY more powerful than any than any single item, because it can get you tons of items.
