i killed a Ultra Greed with The Kepper. Why i have locked this item?

Things which prevent progress/unlocks/achievements:
Seeds.
Challenges (except for their reward obviously).
Daily Runs.
Any active mod from the mod menu (Afterbirth+).

without nothing

Did you kill Ultra Greed or Ultra Greedier? They're not considered as the same thing by the game.
