** edit ** Some fine folks below has pointed out, that there is an .ini file in which mousecontrol can be enabled. **
Since you can't aim the shots, only shoot either up, down, left or right, the mouse would have to do the same.
There is even an item that allows you to shoot along diagonals (up+right, left+down and so on), which is an upgrade.
Many of the bosses are dependent upon you not being able to aim shots with the mouse - they would be extremely easy if you could.

Go see if you can find a mod that binds "Directed tears" to mouse movement :P
There's also the fat fly, and the weirdly-named tech item that gives you a controlled tear.
Go to options.ini and set mousecontrol (or something like that) to 1.
