I have defeat ultra Greedier with Magdalene many times but I can't unlock this item. Why? Can someone help me?

No I don't use any mods and I've also defeat him few minutes ago...

When you start a new run, under your keys, do you seea symbol like a cup crossed?

You have the Glyph of Balance achievement since January.

If you're referring to the Collection Page ("stuff I found"), unlocking an item is not enough to show it there. You have to find it during a run.

Sorry I can't understand what do you mean with "cup crossed"

In the top-left under my keys I see the cent with 0/11...

In the top-left under my keys I see the cent with 0/11...

He meant just below your keys/bombs/coins. If there's a crossed out trophy symbol, it means unlocks are disabled.

You have to fill up the collection page entirely, which means pick up every item during runs (without seed, mods, and whatnot of course). Just find it and pick it up. Items you get from rerolls (D4 effect) count too.
The only item which requires something special to get is the Tonsil familiar. The only way to get it is by triggerring the Tonsil trinket's effect (or starting with it randomly as Eden).

Unfortunately, as far as I know, 1001% (added by Afterbirth) requires all items on the page, including Afterbirth+ ones (if you have it installed of course). But if you're on your way to get the 1000000% (1001% equivalent for AB+), it's just a mild annoyance.

Well, consider it this way: you already did the hard part. Now, it's grinding. Boring, slow, but far easier. :P
