They're the fastest(?) enemy in the game, and some of the room layouts are brutal.
Probably these. They're almost always cramped into a room with barely enough (and sometimes just plain not enough) room to maneuver, the masks are aggressive, and the hearts can defend themselves effectively just to make things worse.
Plus, whenever you kill one heart, it's never the one that's tied to a mask that's currently chasing you down, it's always the one across the room that's not actually a threat.

But the enemy I like least -- and this is quite particular -- are the walking host-hats in that room with two lines of stationary, continuous spitters. I dread that combination more than any other. I just know when I see it that my run is about to get wrecked. Flying/brimstone helps a lot.
Runner-up: eye lasers. Pew, pew!
