Paging Blaine, paging Blaine.
After dissecting the wiki and the official forum for his whereabout, I still couldn't find him. I do remember seeing a locked door that requires 80 hacking but can't recall where was it.

If anyone could help pointing to his direction, that would be much appreciated.

Go north of SGS, head down the stairs, turn south, look for the intercom. You'll need to talk to Old Jonas or people in Junkyard to get the door code.

Head out from SGS towards GMS. The first underground tunnel on your left, where you got ambushed in the jack quicksilver quest, take that. Somewhere in that sector you'll find a door behind another door that's locked. The electronic lock next to that is how you open it.

Best way to get the info from Jonas I have found is to just give him his watch for free. Then you will always pass the persuasion check. He also gives you a cheaper price from then on. Making it a good place to get cheap lockpicks and batteries early on.

I did kill Blaine, but I don't remember who to give his head to, and it isn't in my notes.
Who do I go to to get the reward?

I think blaine is overrated. He's useful for newbies... but I suggest doing the Oculus quests asap once the world is open. It can be done really early as nothing in this questline really needs high skills (as long as you have lockp/hack) nor combat power. And the reward is one of the best traders in the game. minor spoilers:

- Abrams first quests can be done even without any sneak. Fighting also isn't necessary.
- For the core city and SGS quests, you just run around and talk iirc.
- For the foundry quest you either play as a woman to get into the jail (saying you're his wife) or do the beast quest, which can be easily done with some gas grenades on the northeastern chokepoint of the stairs.
Thats it. You can even go ahead and continue with the next quest to get the huxkey:
the hardest part is becoming a gladiator, which can be done with just some molotovs.
