I've also tried all the recommended solutions. Disabling hyperthreading, editing ini, adaptive vsync, setting core affinity, etc. All to no avail.
Interestingly, if I draw a weapon and hold parry, then I can move around and the camera remains perfectly smooth. Based on this, it seems like it isn't an engine limitation, but perhaps a bug that could be fixed in a patch.
This is on a 3770K/GTX 680.
Try this:
Hold your walk analog stick only slightly forward so your character is just walking. Try rotating the camera now... it's totally smooth for me.
Now, move it slightly forward so the character is running. But not all the way, just to the point where he is running. Try rotating now. This is also smooth for me.
Push full forward on the walk stick, and try rotating again. Now it's hitching all over the place.
It reduces the jerking and hitching substantially, even with full forward move stick. It's still there though, just very much reduced.
Definitely something wonky is going on with the game's controller code. Seems like maybe a deadzone issue?
It makes me wonder if there is any third party software for calibrating the deadzone/analog range of XInput controllers.
