Things like firearms, body armor, vehicules and electronics (hacking) become luxury with the Extended mod, you will need more parts to be able to properly make use of them.
For example a Rifle, you want to put a strap on it?
Ok first of all you need a strap on good condition aswell as a screwdriver, if you don't meet that condition then you will have to carry rifles with your hands
want to shoot a pistol?
now the pistol and bullets aren't enough, you need to be lucky enough to find the right magazine for the right gun, if you want to reload that gun you better have a spare magazine.
want to get a shopping cart?

In my opinion, if you already completed the game, got the legendary traits or you already managed to get late-game gear (such as body armor from the DMC guards) then i suggest give it a try for a more "realistic" and interactive enviroment, because the main problem with vanilla is that you can be around DMC the entire game and get every single item you could ever want, with extended you can do it aswell, but with the improved bases you can set up multiple camps and get away from the DMC and maintain a better security than before, it really rewards future planing and it gives you the tools to actually explore the world (food is still a bit of a problem so foraging is always encouraged if you want to get away from DMC).
In vanilla there's a point where you just become god, carrying 60 bullets on your pockets, with 2 medkits worth 6k on your back, a stolen body armor from a random DMC guy that died and a pistol just to supress people, it gets really boring really quick. Extended DOESN'T fix that, but it puts more barriers towards making the player a killing machine, now bullets don't stack, you need to have a bullet case if you don't want to take half your inventory in bullet space, magazines are a thing that limit players from shooting guns, making bows a more reliable but less deadly option, with an extended combat melee gets quite interesting aswell.

But other then that. Everything has been going smoothly
I just shot 2 dogmen 5 times and neither of them died, so, yeah I'd say its harder.
