Dio's Best Friend doesn't stack.

So i was having a really good run, the best I've ever had, and I pick up (for the first time) Dio's Best Friend. I went on to pick up 2 more of them. But for some reason, only one of them was used before my death. I don't know if that's on purpose (probably not), but that lost me a really good run.

If you get another one then they only refresh after you lose one, it does not stack and this is not a glitch
