When you reach the deep chasm; there is a pointless dead-end opening with a mutant visible from it that is very easy to miss and just smoove right past. Its most likely that guy. You most likely got the mutant at the base of this chasm as well as the ones on the way to the crowbar; if that helps you remember where this is.
Thank you for the reply. I did kill that one though. I also made sure to kill each enemy with explosives in case you were supposed to not leave "evidence" behind lol
I had to use DEagle and then I finally got it.
It also seems that it is fine if the flamethrower guys kill them, but I cannot verify that.
The two mutants I missed on my first achievement run-through are
in the crowbar room, if you don't go to the far left corner of the room, one of the monsters will not spawn.
In the main hub, inside the hut at the top (easy to miss because there's no point going inside)
Another easy to miss is the mutant in the dark flickering tunnel that ONLY spawns if you go underneath and crawl towards the shotgun ammo pickup.
I don't know if killing the first two mutants in the level yourself matters, but I did that too.
