You need to obtain all secrets in all those three levels in the same playthrough.
By starting a New Game or New Game + the game will not take into account Secrets obtained in previous playthroughs.
By loading a save file at any point in any of those levels you'll be sent back there and the game will remember your Secret count at the moment you made that save.
That's why it's recommended to keep multiple save slots. Say you have a save from a previous playthrough at the end of Venice with all its secrets, you won't need to start a new game and replay Great Wall and Venice, just load that save and continue from there.
And I can confirm it must be done in 1 playthrough. I have collected all of Egypt's secrets in this run except the flying Uzis in Sanctuary Of The Scion (but I collected on my first run) and no achievement until I re-collect it again.
