40 Fathoms is one of the shortest levels in TR2. Try to save some medipacks there, or next level Wreck of the Maria Doria might be easily a bigger problem.
What your meant to do is flip around, jump voer them ad flip in the air while shooting, run up in their faces and roll so they have to turn towards you, etc. I did TR1 and 2 medipackless on the original Playstation by doing this
I would stil redo the whole level.

Though I guess technically it is possible to never get hit by gunfire, but that eliminates any trace of fun from the game.
If you roll as you hit the ground you can sometimes, if timed right reduce a little bit of the fall damage.
It's not much but it can help.
So using the most effective weapon for the situation makes it a lot easier. For example the M16 outranges the weapons of enemies, so if you can aim at them from far enough away, you can completely avoid any damage. But it's completely garbage in close range because lara stops firing after every jump or step.
The shotgun on the other hand deals massive burst damage close range, so it's a good choice for narrow corridors when enemies are surprising you from behind every corner and there is not much room to run around and dodge.
Magnums and Uzis are best for medium range fights where you need to run around while shooting to avoid damage.
Because of this I would recommend to try to keep your ammo count balanced across weapons as much as possible so that when you come across a tough enemy, you always have at least some ammo of the best weapon available. And if you're overall running low on ammo, there are a lot of enemies that can be safely killed with pistols only. For example animals after jumping on blocks where they can't reach you, fish and sharks from outside the water (might require you to jump into the water to lure them and then climb out to shoot repeatedly though).
