EDIT: oh yeah! Que the "I did it first try" people.
Beyond that I don't know what to say. I've heard many reports that the positioning is janky, but I never had much problem myself. Though ideally you want to kill it away from walls and the central area, because the water holes can block access, and walls will sometimes force you to run all the way around the dragon as its head is blocking, by which time it's too late. But if it's in a good place, I just try to position myself as centrally as possible to where the dagger is, and if it doesn't activate immediately I just run into the dragon whilst turning a bit to move position slightly until it does. It would have helped if they put an action indicator on it though, since there doesn't appear to be one. That way you could see if the positioning is correct.
No but my wife does when she's angry at me. Hey, I take it as if: I'm off the hook cause I don't understand and can then plead my case legitimately. lol
I've no idea how you're supposed to win that fight without using the dirty "stand behind the pillar and shoot grenades at him" trick though. I always end up in flames if I try anything else.
This is the best walkthrough I found so far to win this fight.

I actually tried it with modern controls to test the keys / buttons for removing the dagger, and if anything it felt slightly easier to fight, since if I could get alongside it facing towards its rear, I could keep circling so that it was always trying to get its head round to meet me, not giving it much angle to breathe fire.
This. I can't really remeber much about TR2 back then - but this was an issue in the nineties already. Without external help from some game magazin or guide I couldn't have done it.
If the issue is still there, then this is a sign of a high quality port. ;)
But I guess in this case it would make sense to have it fixed - at least as an option.
