Tomb of Tihocan secret bug
Did it 2 times, just to see if its really a bug or not.
1st time i did it by mistake, and the 2nd time on purpose just to see if i can reproduce it.
i-ll add spoiler tag, just in case.

so the 1st secret is in the room with the pressure plates. and 1 alligator with some ammo pickups.
obviously if you first go to the left side (instead of right where the french guy is) and activate the lever to fill the room with water, the room will remain that way, even if u try to pull the lever back, the water won't drain and the secret is impossible to get - unless you restart level. it-s not that much progress into level, still - kinda annoying

yep there are anti-triggers that close off secrets if you go a certain way before another. it's not a bug, it's by design
