It's also possible that the devs simply chose the name Curly for him first and then added in the other name (partially obscured by the damage) as an afterthought. Or perhaps he signed his ID last name, first name for whatever reason. Curly's ID, for reference.[mouthwashing.fandom.com]
EDIT: So I did some searching and apparently it's fairy common in certain UK spaces to refer to people by their last name for a few different reasons, with respect to one's rank or status being one. I bring this up because the devs confirmed that they originally intended for Curly to "have a British accent" (and he speaks like a Brit in the game). I was originally thinking about America/the west, in which you usually call people by their first name there.
I was curious so I skimmed a doc[docs.google.com] of the MW script and counted how many times the crew refers to Curly as either "Captain" or "Curly", hopefully I didn't miss any...
