1. Start out with some simple made foods
This comes pretty easily since customer patience is shortened in extreme difficulty. I usually start with foods like corndogs, pretzels, beer and coffee (the last being under special foods which you can access right away on extreme). Make sure you stay away from fried foods (e.g fried chicken, fried chips and sopapillas) in the early game as the time it takes to prepare them will leave you with a lot of other customers leaving the store.
2. You will most likely always have a day with 70% buzz+
The main thing to consider is that every day on extreme will have lots of customers. Due to this, it may feel as if you are doing a lot worse, but in reality you may only be doing around the same percentage of bad/average orders.
3. Hopefully, you will be getting more money
You start out with 15 000 dollars on extreme difficulty (double tthe amount on normal) so don't be afraid to experiment! Find which foods work best for each situation and adapt!
Hope these tips helped :)
