Amazon's "redo battle" not applying bonuses (dungeon)

The Amazon had the colossus as a follower and the oaths of vengeance. Upon encountering the battle hulk I rolled a 1 the first time and chose to redo battle. However for the 2nd battle, her follower and item bonuses were not granted, only base strength was shown.

How do you even choose to re-do a battle as an Amazon? I can't figure out how to use the ability.

Skorj after standing off or losing a battle (strength only) just before you click Confirm or Continue (to end the battle) it will be under there as Re-Do battle

I got something similar playing a game last night; I was in the dungeon, getting +3 on battles, but when redoing them I was only getting +1. I can't remember exactly what stuff I was carrying (I kept dying so dropping everything...)

Also: I had a couple of times where I lost a battle, re-did it and won, but didn't get the achievement. Does that only unlock when fighting against another character, or is it any creature?
