Anyway the Nether Realm just adds one new deck and 3 new endings, so all the new cards are related to the new endings.
OFC this are killer cards and you dont get a new char or something else, so its on you, as i like the new endings i would buy it.....
If you love Talisman then you need NR, but the question is how high priority it is for you in comparison with other released expansions. It is micro (not even mini) expansion, so (for digital game, which has no some of restrictions of physical game) probably its better to buy bigger expansions first (starting with Highland - it easer than Dungeon and is very fun and thematic), and mini expansions after them (starting with Reaper or Frostmarch (because of Warlock Quest cards)).
The deck is used for the three endings in various ways: Pandora's box allows the first player to reach the Crown of Command to take command of this deck and use it to decimate his opponents. The Gauntlet increases the difficulty of Inner Region by forcing the players to overcome the Nether cards placed there in addition to the normal challenge. Finally, The Hunt is a collector ending (in the vein of Warlock Quests or Sacred Pool) where you can choose to replace your Adventure cards with Nether cards and you need to defeat 4 Nether Enemies to qualify for the Crown.
Its really a personal preference thing if your the sort that loves the thrill of danger surpassed and a win against tough odds then its a great one. If you want to get great prizes designed to make you a powerful hero its one that you might want to skip.
I dont object to the difficulty of the Nether Realm cards. I'm also not bothered by the lack of 'goodies'. I'm bothered by the fact that the only way to use the Nether Realm cards is by using one of the Nether Realm alternate endings.
so if you like playing with random endings, you wont be able to use the nether realm expansion. ANY of it since it doesnt even offer new characters to play. I think a lot of players out there simply assume that expansion = new cards and new characters that 'grow' the base game, not simply 'replace' parts of it.

now having said all that, im finding 'the gauntlet' ending to be quite fun and worth the price even if i dont use that ending every time. still its a REAL shame that i have to choose between nether realm cards and battle royale, you know?
Yes, though I think that's a general added functionality and you don't actually need the expansion for that.

Also, Pandora's Box might need a little tweak: reshuffle the Nether discards before the character draws the cards for the ending. Otherwise, he might not have enough "material" available.
