Regardless the answer is simple, enable all DLC`s.
Firelands, Blood Moon and the Dragon really make the game more hectic. I usually only activate one when I want it to be the focus. I'll only activate them all if I want the game to be rather chaotic.
Dungeon and Highlands don't interfere too much with the base deck or standard gameplay but they offer alternate challenges that will split up the players. That works particularly well if you pick an especially hard ending (from the Nether Realms extension for example) and you want areas of greater average challenge. They're not equally hard areas, so you can include them both for more choice, or stick to just one if you want to promote more player encounters.
They work well with any combination, and can even provide some breathing space from the madness of the Firelands and such.
My most common conbination is Frostmarch, Reaper, Sacred Pool, City, and The Dungeon. Although I only pick The Dungeon over The Highlands out of habit, I should probably change that.
I personally tend to play with all three extra boards (City, Dungeon & Highlands) along with Frostmarch, Reaper (-Reaper), Sacred Pool & Blood Moon (-Werewolf). When The Wildlands Expansion is added I expect that to be a "permanent" addition.
Note that The Nether Realms is pretty much pointless unless you use one of the three endings as there are no characters or base Adventure cards in it. But it can add an interesting twist if you do use one of the endings.
As to what dlc to include when playing with the dragon, doesn’t really matter as that expansion dominates all boards.
Thus far the only expansion I would look at dropping is Dragon. So many token draws, and so much extra stuff to keep tabs on. It feels like too much for a fun roll-and-move game.
I understand the dislike for the dominance of Flail (would be happy to see the electronic game diverge from the board game here), but I feel City allows players a bit more control and strategy, so I always keep it on.
I've actually considered disabling Highlands as it feels like the region is just better than the main board in most situations. I also considered disabling Dungeon just because the AI loved running off to its death in there.
