Report a translation problem
Each time you input the right number, the door screen will flash green for a second.
Input the wrong number, the door screen will not flash at all.
Example: Door code is 5746
Input: 5555 ---> Outcome: The 1st number flashes green, the rest don't do anything.
---> Remember the 1st number is 5
Input: 7777 ---> Outcome: The 2nd number flashes green, the rest don't do anything.
---> Remember the 2nd number is 7
The rest follow the same method.
If you type in 1111 and there's no flashing green during the typing, it means there's no number 1 in the code.
Unless your game lags when playing the game.
Tried this on the door in Evil Science and it doesn't work. The door on the screen is completely red the entire time.
Yeah, some doors are just locked... there's nothing behind them.
The only doors you can open are the ones where the console screen is clearly green and you're able to press it and well obviously the door will then open. Other doors are locked by a keypad and Linzombie already explained how they work, if the screen is red... the door won't open... because the developers don't intend for you to go that way.. because there is nothing beyond that point.
Because the door is locked, you can't go that way, there is nothing past that door.
Doors that have a keypad on them, like there are multiple buttons you can press those are locked but they can be unlocked by figuring out the code. If you don't see a keypad... and the console is red, then you can't go that way.
Yes, there IS stuff behind that key padded door to the right on the second floor and the key pad numbers trick did not work as advertised. I was very clear in the description of my comment that the trick did not work on a locked key pad door. Still you choose to assume that I am an idiot so I am writing this comment to further dispel any doubts that I tried the trick on a correct, key pad locked door, on the second floor of the Evil Science assignment where the locked keypad trick did NOT work to unlock the "morgue" area without a fully correctly entered password. The lock never once flashed green, it was a complete and solid red the entire time up until the correct pass code was entered in its entirety to fully unlock the door.
I JUST tested that door. The hacking worked as intended.
Therefore, something's wrong with *your* game.
The green flashing only appear 1 frame.
Low fps can cause the game to "Skip Frames".
(Because this used to happen in my old computer.)
Right okay so you're talking about the one upstairs, you should have clarified that in your original post. At this point I'm convinced you're just doing something wrong.
Only other thing that might help is like Linzombie said, possibly something to do with frame rate. But I run vcd at 120fps and still never had an issue with keypads.
So this whole thing is strange, either you just don't understand how solving door codes works, or there is something completely broken with your game.
If you're on a Windows PC, try running the 32 and 64-bit versions to see if there's a difference. If nothing else works, I would suspect it's the framerate, as the others have suggested.

But if you're hunting for achievement, I suggest you "GIVE UP". (Or use "cheap Workshop maps" to get them.)
If you don't know how the hacking in-game works, then there's no way to get "Clean Freak" achievement. Or even get any achievements related to Bob. (Since 2 of the Bob notes are locked behind the Office door.)
Hell, not even the "all special collectible" achievement is available for you!
(Which really sucks for Mac users.)

If you do, try to record the process. No need to be long. Just the footage of you pressing the code.
Evil Science door code is 2199. Try to record you pressing 2222, 1111, 9999, then 2199.
The footage should uncover your problem.
Why people even use mac to begin with is the real question.
