And if the beeping thing beeps loudly/often, there IS definitely something near you, you just gotta figure out where, sometimes it's a hidden away piece of junk, or bullet hole, or worst-case scenario, a REALLY hard to spot faded spot of blood/soot that's almost gone, but not entirely, which should be gone if you frantically mop the local area that beeps, if nothing else could be found.
As for the notes, it's not the inspection's notes that give you hints, but news articles like 'barrel slays worker', etc, which hint at the various messes/objects you should have dealt with, they typically have images, too. If you really want to get a high score, you should check the floor and see if a level has any spot marked with lines in a square pattern with names like 'Barrels', 'Crates', etc, and fill them up with the relevant items.
That gives you bonus points (which can exeed a 100% score), taking the victim's PIDs (looks like USB sticks kinda, shows their name when picked up) and shoving them against the Punch-Out machine adds them to the list of casualties which should be on the bottom of the report (which you can optionally fill in for bonus points), letting you click on their name and fill out in what state of death you found them, etc. All optional but good when desperate for points.
And also, the game's just really harsh on what is good enough to NOT get you demoted (like something above 90% or 95% ougta get you not demoted), not that getting demoted does anything... Getting fired, though, is bad and resets your office, removing any items you've collected and brought to it via the Janitor's trunk as you punch out.

Problem 2: There's a hole in the door, right at the seam and the top edge of the glass window. Note the dark vertical streaks - that's the decal all stretched out.
Edit: Also in Problem 2, there's a hole in the news ticker right in the middle, under the word "recurring". Also, where the heck are all the crates?
The others i can't entirely see what's wrong with em, but there's SOMETHING hidden there, i'd imagine, if it's beeping in blue mode, maybe a vague bloodstain or tiny gib, in green mode, perhaps a bullet casing or bullet hole, or stray piece of trash (or even a very faded out soot mark that needs mopping).
Always something, doubt it could trigger without a mess nearby, that said, though incredibily rare, said mess could be stuck in the level and hidden away. If all else fails, the 'killnearestmess' command can deal with those effeciently.
I'll be more watchful in the future. :)
In screenshot problem1 I did as you said, fixed the door control console. But its still beeping. I used the command 'killnearestmess' 10 times and its still beeping. (green)
Also as you can see this 'cauldron' is beeping as well with the green indicator... I just can't figure out what it is that needs fixing as I can't see anything that needs to be fixed. :S
It only beeps on that side like shown on the screenshot and not on the other side of it.
This really confuses me :S

Seems to be coming from the inside maybe? No idea how to fix that as the fire obscures view of everything...
It's entirely possible that there's a mess inside the furnace. That IS a bug. Swab the inside with the mop and see if that helps.
