Poor story telling. I blame devs.

No, poor user understanding. A user's skill issue.

There are tons of fantasy games and therefore you can't blame users, if they get confused.

The game did a poor job during and after the "big reveal", even many fans/defenders and journalists didn't get that it was most likely just an inner journey.
They thought it was more metaphorical and Senua was about to become a real leader, but that doesn't make any sense.

I think nothing in the story is actually happening, probably some kind of inner journey, while the companions she meets are maybe the voices inside her head, while the giants and draugr she kills are basically accounting for the past (first giant could be her mother, the leader her father etc.)
Not her mother, never her mother. She dealt with issues of her mother in the first game (Hela's burned appearance is based on how her mother looked after being burned at the stake and the faces mechanic originally called her mother forth to support her).

The first giant is her hyperfixating on the empty cradle in the ruined village she passes through in the beginning as a representation of the loss suffered by the people of this land (not made easier by the Draugr worshipping the volcano, who she only sees as monsters because they're actual cannibals as Fargrimr helpfully explains)

It's why Illtauda bends her head like that - pareidolia of whorls in the volcanic smoke/ash, just as much of the second giant is of lightning strikes highlighting rocks (which he turns into when he dies) and sheets of rain while his appearance in the water is pareidolia of the waves, a-la Loch Ness monster (remember, Astridr points him out, so Senua isn't the only one "seeing" him). If you pause the game in photo mode during the cave sequence when she and Astridr's tribe attack the giant in the dark, he's only there when Senua looks back to check on him. Same reason why we only ever see the giants hurt by spears that are thrown either by Senua or from off-camera.

I wonder how the volcano sequence looked like for Fargrimr and his tribe though, because she clearly didn't just sit there meditating the whole time.

See it's all up for discussion, haven't given the story much thought after finishing the movie, eh game, but I wasn't impressed overall.

I mean you have probably thought much more about it and still haven't figured out if anything ever happened in reality.

The most likely answer is that the giants were never real in the literal sense. They were an example of a shared delusion based on fear, ignorance, and unexplained deaths. Perpuated by the slave leader and others. That's why the 3rd giant never appears, because it was the slaver leaders story. The only question is whether when they "fight" the two giants, are they fighting real humans that they perceive as giants, or nothing at all. Are the "giants" even present during the conbat or are the others just running from nothing? Could be either, maybe a bit of both. When the giants turn into stone, that's effectively the shared delusion coming to an end. Senua was able to help because she knows how to navigate delusions in her own mind, and she is bringing that skill to others.

Remember that how she defeats the giants is by finding their name, which represents actually defining the delusion but also breaking the delusion with factual history that started the whole thing. The hidden ones in this context might simply be people that do not see giants and are "ignored" by the others. Or it might by a representation of senuas mind putting together bits of lore she sees and hears to find the "giant" names from locals, and these parts ( e.g. puzzles) are probably not real.

If the story was all in Senua's head i think the writers would have tried to hint at that in the end, maybe by having her wake up on the beach shipwreck, but since they didn't go that route it most likely wasn't just Senuas mind.

Given it was all a delusion, I wonder why they didn't opt for 3 different types of "monsters" but i guess that comes back to it all being driven by the big jefe and the region/ mythology.

Edit: now that I've looked at the story a bit closer I appreciate it more and I think it's clever to make the giants seem real.

Posts like this show just how many people are completely lost as to what this game is trying to say. Clearly the story went right over your head.
Poor story telling. I blame devs.
Just because you can’t grasp concepts that aren’t spelled out to you phonetically doesn’t mean it’s poor storytelling. Try thinking about what you experienced in different way instead of just believing what’s directly told to you.
