In terms of quest results, as a general rule of thumb most big quests have both a kill option and a save option. So you can be fairly murderous in many quests.
To my knowledge you won't see any ultra evil things in this game though.

What can you become truly demonically evil? The same scale of this? You can become the Lord of Murder at the end of this game.

You can't just be a serial killer because you are a character with a job, like Shepard from Mass Effect. You do get way more choices about who you are as a character but you can't just free form it.
I think it's really freaking great, especially coming off finishing PoE 1 and 2 back to back.
I know rpg gamers can get really picky about what they want in a game so if you need murdering everyone to be an option you will be disappointed, but it's not like that was an option in either of the previous Pillars games.
You can make some very evil choices in this game that do have lasting impacts though, more-so than in either Oblivion or Skyrim.
