Well on the topic of 1 I can add something. So imagine you have a 1GB file that contains some map data. Now someone changes just 1 Line of code in that. Could be something super easy. Turns out the entire file has to be re-downloaded. Thats why these patches in many modern games are so huge. The files are huge and any small changes... means you have to download it again. Not the best solution but that is the way it is. And that is why in older games the updates were smaller. The files were smaller. That is not up to the devs but that is how steam handles updates.
