You know if good karma is a thing, such things inherently wouldn't happen to these zoi's, as if the reliability of their lots items are bulletproof, compared to those with a bad karma rating where things don't stop going wrong no matter what.
I had the exact same thing happen to me, played an hour, on the end of the first day, during his first night, he died from natural cause.. my karma was good, he was more than healthy, not entirely sure what's going on with this feature but it needs just overtuned as of now. They need to either fix this or give us control over it because right now it doesn't sound right
And imo it has no business being in a life sim in the first place. The Sims has random death as a meteor drop, but it's very rare and avoidable.
Go back a save and play an other family until your main Zoi should be dead until after a day. Than look if the Zoi still lives. I bet it does.

you know, maybe being able to pay for an autopsy in the game might be cool
