its not bugged. if you attack him, he just reflects the dmg. the ability is active, if he spawns.
you have to wait 1 turn, till his ability turns off.
so either unequip demonic for kotya or click wait button.

yeah you need some kind of defense. if there are 3 of them its get even harder because u cant stack sparke stuff. :/ some plain armor would help or scout set i guess.
turtle set could work too.

The best counter is the turtle set, it avoid all damage in the first turn.

The only way to somewhat painlessly get around this enemy is to always keep one item of that 'negates all damage in first turn' set in your inventory. Then have one of the three sets you are wearing be able to be swapped out for that set.

So for example lets say your main set is Demon + whatever + you have a weapon with the double attack set. You would want to have a backup weapon you could swap to with that 'immune first round' set (forget its name) so you could swap that in. Now you can blast and not worry about it. It is the only somewhat viable workaround because the alternative is removing your entire demon set which is dumb as ♥♥♥♥ since that set is OP and probably something you are investing into with multiple pieces.

edit: probably the ideal would be to have your boots or a ring be a single item for the third set you use and then you could swap that out. I simply used weapons as an example.

Forgot to add~ this is because as you said waiting on turn one will get you killed in later floors. You can maybe get away with it sometimes if you get lucky but I found I had to get a better workaround because floor 20+ I would die every time if I tried to just wait turn one for Kotya parry to vanish.
