I took a fifth screenshot using the ALT-view outside of steam. Do you have an email address I can send it to?
From the ALT view, the only unexplored tiles appear to be in the guard elevators. Other than that tile, the grey border is unbroken around the entire level.

In other news, I just rescued a Courier, and the item I gave him to assist in our escape despawned after the level. Is that intentional? Shouldn't the item go into my general inventory when the mission is over?
For the Courier -- yes, that's intentional. He ran away with your goods. We could make it go to the general inventory, though we'd need to decide what would happen if the inventory is full.
Ultimately it's a design decision on your part, but I'm not sure what it adds to the game to have to remember to click the "transfer items" button before the "exit" button once you're all in the elevator. At that point encumbrance (?) isn't relevant, because everyone's already in there with him anyway. And there's no reason to rush yourself, because it's a turn-based game. Maybe there's a strange edge case I'm not thinking of...?
