Secret Room?
I broke a wall and it lead to a secret room.
is there a way to tell what walls are breakable? I was in a room with Elite/difficulty affect, is it only on those levels?

nah, they have existed long before hard mode. you can tell by the fact that there's a secret room if there's an adjacent room right next to your current room or very very hard to notice cracks on the walls.

To me, this is a problem as my eyesight is bad. There should be something that makes it clear that there is an entrance to another room that is not marked on the map.

There are quite visible differences in the pattern on the walls where a secret room is. If the room is on the left/right or bottom the difference will only be visible in the top border of the wall, but if it's above then the wall itself and the top border have cracks.
@Paw maybe when you left your initial reply the difference wasn't very noticeable but it's quite obvious now, possibly a patch/update made it more obvious

I found another one that has an easier tell: I could see a room on the other side of the wall lol.
though sometimes that's just a regular room - not a waste to check though.

yeah, they've done a few patches since that makes it way easier to tell.

If all else fails, there's the tried and true method of just swing at a wall on each end, since there a noticeable hit indicator when you do hit one. It don't take very long to check thankfully if you not 100% sure (less you playing the mole, yikes, dash a lot)
