If you acquired an “Attack Achievement Effect” (or other Achievement Effects) does the associated Passive Skill have to be equipped in the “Quick Slot Set” first to activate the Attack Achievement Effect?

I noticed when the associated Passive Skills are not assigned to a quick slot set, my character has combat mechanic issues during game play; as if the codes are looking for a way to apply the effects but can't because the passive skills are not equipped in the Quick Slot Set so my character just freeze in place.

Yes, mastery skill modifiers upgrade the slotted passives.
