Cannot get the codex when you open the Loot Chest and then pick up the Fallen Paper.

I have the same issue. Were you able to finish the quest since then ?

UPDATE: I figured it out. It's not a bug but it's very misleading. When you open the chest, a "Fallen Paper" spawns next to it and that's what you actually need to pick up.
