I do believe they have stated that most things you do if not stealthy will attract/lure cannibals in ways, the mechanics are meant to be that they are aware of your presense and are trying to seek and hunt you.
The AI isn't fully implemented nor near finished yet, campfires attract them already and so do your structures. You should probably read up on how they are going to make the AI more intelligent and how they plan to make your actions have more consequences before saying "it doesn't work that way" because it sort of does already, even if minimal, and will do a lot more in the future.
nothing i've played has driven me to a point of paranoid insanity if that's what you're asking. i'm either too desensitized to the stuff or nothing has really tapped into what scares me the most.
in terms of best quality and overall production, Amnesia would be #1 because of how well made and community friendly it is. most of the custom storys have been a better experience than the cannon.
anna, and Anna the Extended edition really knows how to bring in the atmosphere i get more goosebumps playing that than any other, though the item searching and lack of direction kills it a little.
there have also been a few free indie games like The Train and Kraven mannor that had a very nice use of mannequines that threw me for a loop once or twice.
Deadspace 1 (in the years before sequals were thought of and before it turned into a generic shoot-em-up thriller) set the bar for a new horror genra that year it was released.
penumbra and Nightmare House 2 were also good half life mods.
Alien: Isolation looks right up my street :)
