2nd death is game over.
In a single session on single player you normally get to die twice. The first death sees you drug to the cave, second death results in a game over and a reload if you want to continue. You can change that though. When you reload that save you also reset the current death counter. Meaning, even if you got drug to the cave before, if you have saved and reloaded since that happened you will be drug to the cave again instead of simply dying the next time you go down.
