Megan spawning outside.

As the title says, is this supposed to happen?

After you complete the alternate ending, a version of megan spawns on the surface. It looks similar, but has significant differences. For one, the body is pink instead of blue. For two, instead of megan's body underneath the mutant parts, it's instead a Worm in between controlling it. When killed, this worm drops from the body and if not killed, will start constantly giving birth to more and more worms till it becomes capable of attacking. This is why I call it "Worm Megan". For three, Worm Megan has less HP than the endboss, but is still quite dangerous now that they're no longer restricted to one flat plane. And finally, they drop 3 peices of creepy armor upon defeat, and an additional reward being a lot of bones if you let the worm duplicate a bit and then kill it to get bones from its body. Quite a challenging mutant, if you ask me.
