Left click on body parts,sticks,bones look at your custom effigy to start adding stuff to it.

you have to get below 90% sanity in order to see the effigy page, which can be easily achieved by cutting up bodies and/or eating them. Once you are below 90% sanity the effigy page will appear right after the signals page in your survival book. if you use one of the already established effigies it will tell you what parts are needed for it and you can add those parts, if you have them in your inventory, by simply walking up to it and pressing E. if you are using the custom effigy you custruct the pole itself first, then you can add parts as you see fit by holding that body part in your hand, looking at the pole, and pressing E when that body part's outline matches the place you wish to place it. You can even put torsos on the custom effigy once you have cut off all their limbs, but for the heads or limbs themselves you would have to select those items from your inventory so that you hold them in your hands befoe you can place them.
