Submerged Cave completion

I've been knocking all the caves off my to-do list, but I can't seem to complete the Submerged Cave despite going through it several times. From what I can tell you need to find all the hotspots in a cave to complete it (the areas with the musical stings). Are there any more hotspots than 1) the dead passenger in the first room, 2) the rebreather room, 3) the platform with the babies on it, 4) the giant door, and 5) the ledge with the photo near the end?

I'm having the same issue, I think it might be because in earlier versions there was a map piece in that cave in one of the tents by the skeletons and tents, and since they took out the finding of the 4 map pieces and closed off the tent that had it, the game might still think it needs to be collected in order to fully complete it in the to do list. that or we are clearly missing some secret area of the cave.

You can open that giant door if you put some stones on the pressure palte opposite to the door.

Has anyone found out why it's not unlocked in the book? Will I need to create a new game to get it?

I already have the spelunker achievement so i can't test specifically that but, I did at least test out the submerged cave and it triggering. I was able to get the initial "explore submerged cave" prompt the moment I found the squeeze entrance, however I went through once, making sure i got every story related item along the way and all main areas...and nothing. Went and got the map, did it again (a lot more sloppy no less, wasnt checking every edge and had already picked up the story stuff so didnt grab even the parts of it that do respawn like the toy piece), and on that attempt it triggered as completed as I was in the water tunnel between the rusty axe chamber and the final campfire before the last pool.

It may be possible you need to have the map on you before exploring the cave, to get it to properly trigger. I admittedly got spelunker a while ago but i remember I did a single player game specifically to use the "drug to hanging cave" death mechanic to immediately get the map on that run as well.
