Tolkien's son found a Middle-Earth map of the Fifth Age. Tolkien only produced notes about this last period, but for what we know, at this time, magic has completely disappeared, elves, dwarves and monsters are legends, and, in fact, Middle-Earth was transforming to become actual Earth.
If you want a geographic mark, the Mordor was partially flooded as it was becoming Mediterranean sea. But at this point, Tolkien didn't write much more, so all we have is what his son found.
