What does this item do, or what is it used for? I can use it on myself, but I don't see any differences.

Didn't save the playthrough where I got it. I ended up sniping the raider before the monk blew up.

But I think it was a guy in Desmona. David maybe? Something like that.

It will be pretty obvious when you get to the area. All I'm going to say is that there is a quest that leads you to where you can use the locket.

David is in the water company building. His name might be misspelled Davis if you examine his desk and it tells you what the nameplate on the desk says.
